Primary Care, without the rush.
Meet Snowfall Health
Amber Snow, MSN, APRN, AGPCNP-C

As a Primary Care Nurse Practitioner, I've spent my career caring for people through every stage of adulthood. Countless times, patients thank me for the time and attention that I give them, which I feel is the bare minimum of the job. So often, patients leave medical appointments feeling confused, rushed, unheard, or overwhelmed.Â
Â
That's why I've created Snowfall Health.
Â
Primary care should be built on relationships— not time limits and visit quotas. When you have the opportunity to sit down with a provider who listens, asks questions, and understands your goals, healthcare becomes more than treating illness. It becomes a partnership. Whether you're coming in for preventive care, managing a chronic condition, addressing a new concern, or simply wanting a provider who knows you beyond your medical record, we'll work together to keep you healthy and thriving. I'm here to help you feel informed, supported, and confident in your care. My recommendations are guided by the latest medical knowledge, but every decision is made together—with your values, priorities, and lifestyle at the center.
Â
Building Snowfall Health is about creating the kind of practice I would want for my own family: one where patients feel welcome, respected, and genuinely cared for from the moment they walk through the door.
Â
Thank you for considering Snowfall Health. It would be a privilege to partner with you.Â

About me -Â
I was born in Montreal, Canada, and I’ve been in Vermont since 2003. Vermont is truly my 2nd home.Â
Â
I started my career in healthcare in 2009 as a Licensed Nursing Assistant while I attended the University of Vermont for my undergraduate degree. This provided me with early, hands on experience, and where the foundations of my understanding that we must treat the whole person were built. I graduated in 2011 and started working as a Registered Nurse on the inpatient cardiology unit at Fletcher Allen Healthcare, what we now know as University of Vermont Medical Center. I always planned to take the next step to become a Nurse Practitioner, it felt like a natural transition. While in training at The George Washington University, I continued working with cardiology, did clinical teaching, and spent some time with the Vermont Respite House. I graduated in 2015 and started my career as a Nurse Practitioner (NP). As an NP, I have worked in clinics focusing on cardiology, geriatrics, pain management, and primary care. The variety has allowed me to soak up the knowledge that comes with each specialty. Â
Â
I’m in the throws of motherhood, with a bright, funny child that always keeps me on my toes. We tend to enjoy all sorts of summertime outdoor activities - camping, gardening, biking, hiking, swimming, fishing - and really haven’t figured out our joy in the winter activities arena yet. We travel, spend lots of time with friends and family, have game or movie nights, and, on the occasion, you will find me playing some Minecraft.Â
Â
Thank you for taking the time to get to know me and my vision.Â
